The Bishop Hartley Health Program

In accordance with the Catholic Diocese of Columbus and to be in compliance with school health regulations please refer to following list of requirements:

• All students must have completed the Emergency Medical Authorization Form, available on FinalForms, on the first day of school.
• All incoming freshmen and others who are entering Bishop Hartley for the first time will need to provide documentation of all state mandated immunizations. A list of required as well as recommended immunizations can be found HERE.
• All new students, incoming freshmen, and any student participating in sports must provide a recent (within the past 12 months) physical exam form. OHSAA sports physical forms can be downloaded from the school website under the Hawks HQ page. Click on the link to the Ohio High School Athletic Association (OHSAA).
• Students who need to take medications at school will need to provide a completed Medication Authorization Form, which is available below, to the nurse before any medication can be dispensed.
• It is the parent’s responsibility to advise the school nurse of any health concerns or special needs that might impact the student’s health, well-being or academic performance while here at Bishop Hartley.
• Students who are participating is athletics at Bishop Hartley must complete a Preparticipation Physical Evaluation.

Please contact school nurse Mrs. Regina Hinterschied, RN, BSN with any questions or concerns you might have. She will be available at the school beginning August 23 and thereafter on Tuesdays, Wednesdays, and Fridays and at ext. 10629.

Below you will find the necessary forms required for students with certain medical conditions.  These forms are available here for your convenience and can also be obtained directly from the school nurse.  The general Medication Authorization Form can be used for medications related to injury or illness such as antibiotics, ibuprofen, and/or acetaminophen but please note a healthcare providers signature is required for ALL medications, this includes all over the counter medication as well as prescription medications.
